Service Description Funding Sources
Home and Community Disaster Preparedness Provides individual home and neighborhood assessments for disaster readiness and resistence. Volunteers assist elders and families in preparing their homes for natural and man-made disasters. AC
Home Delivered Meals Complies with Dietary Guidelines for Americans and provide 1/3 daily RDA for individuals 51 years or older as established by Food and Nutrition Board of National Acacdemy of Sciences. CCE, LS, OAA, CS, HCE
Home Health Aide Service Personal care services such as bathing, dressing, eating, personal hygience, toileting, assistance with physical transfer, and others are performed by a trained home health aide or certified nurse or licensed therapist. CCE, CS, HCE, LS, OAA
Home Injury Control Services aimed at preventing or reducing physical harm due to falls or other preventable injuries of elders in their homes. OAA
Homemaker The accomplishment of specific home management duties including housekeeping, laundry, cleaning refrigerators, clothing repair, minor home repairs, assistance with budgeting and paying bills, client transportation, meal planning and preparation, shopping asistance, and routine house-hold activities by a trained homemaker. Housing Improvement Provides home repairs, environmental modifications, adaptive alterations, or installing security devices. AC, CCE, LS, OAA, CS, HCE
Information An access service and is defined as responding to an inquiry from a person, or on behalf of a person, regarding public and private resources and available services. CCE, CS, LS, OAA
Intake Administering standard intake and screening instruments for the purpose of gathering information about an applicant for services. It also encompasses the follow-up of individuals waiting for services to review any changes in their situation and ensure prioritization for services. ADI, CCE, CS, HCE, LS, OAA
Interpreter/Translating Oral and/or written communication to non-English speaking and/or persons with disabilities unable to perform the functions. CCE, LS, CS, OAA
Legal Assistance Legal advice and representation by an attorney (including, to the extent feasible, counseling or other appropriate assistance by a paralegal or law student under the supervision of an attorney), and includes counseling or representation by a non-lawyer when permitted by law, to older individuals with economic and social needs. Protects against the loss of basic civil rights and ensures the autonomy and independence of older persons. CCE, LS, CS, OAA
