Community Care for the Elderly (CCE) Program

The Community Care for the Elderly (CCE) Program provides community-based services to help functionally impaired older people live in the least restrictive yet most cost-effective environment suitable to their needs.  Eligible clients may receive services, including: adult day care, case management, chore, companionship, escort, emergency alert response, home-delivered meals, homemaker, housing improvement, pest control services, personal care, and specialized medical equipment and supplies.

The primary purpose of the CCE program is to prevent, reduce or delay premature or inappropriate placement of older persons in nursing homes and other institutions. Individuals must be age 60 or older and functionally impaired, as determined by an initial comprehensive assessment and annual reassessments.
